✅ Qualifications
✔️ Master’s Degree in Social Work (MSW) required
✔️ Active NYS Licensed Master Social Worker (LMSW) license required
✔️ Minimum 2 years clinical experience (nursing home preferred)
✔️ Experience in survey/investigation work preferred
✔️ Must be able to travel and work flexible schedules
✔️ Must obtain SMQT certification within 12 months

✅ Key Responsibilities
✔️ Conduct nursing home surveys and complaint investigations (abuse/neglect cases included)
✔️ Evaluate long-term care facilities for regulatory compliance
✔️ Prepare Statements of Deficiencies (SOD) and written investigative reports
✔️ Participate in emergency surveys and immediate jeopardy investigations
✔️ Conduct interviews with staff, residents, and families
✔️ Enter findings into state and federal reporting systems
✔️ Support monitoring and compliance of corrective action plans
✔️ Testify in administrative hearings when required
