Description
seeking a compassionate and highly skilled Nurse Practitioner or Physician Assistant to join our team in the Skilled Nursing Facility/Inpatient Geriatrics setting.

This is a full-time position with our Geriatrics team consisting of 9 providers (5 physicians, 4 APPs).

The ideal candidate will be passionate about a teaching position in a collegial, faith-based environment and will have a heart for the senior population. As a Nurse Practitioner or Physician Assistant, you will play a pivotal role in providing comprehensive healthcare services to elderly individuals, promoting their overall well-being and improving their quality of life. There will also be potential for teaching PA students as they rotate through the division.

Responsibilities:

1. Patient Assessment: Conduct thorough assessments of geriatric patients to identify their medical, psychosocial, and functional needs.

2. Care Planning: Develop individualized care plans in collaboration with the interdisciplinary team, tailored to each patient's unique requirements.

3. Medical Management: Provide primary care services, manage chronic illnesses, and offer acute care interventions as necessary.

4. Medication Management: Prescribe, monitor, and adjust medications to optimize patient health and well-being.

5. Health Education: Educate patients and their families on disease management, preventive care, and healthy lifestyle choices.

6. Coordination of Care: Collaborate with other healthcare professionals, including physicians, social workers, and therapists, to ensure comprehensive and holistic patient care.

7. Geriatric Expertise: Utilize specialized knowledge of geriatric medicine to address age-related health concerns and promote healthy aging.

8. Home Visits: Conduct home visits to assess the living conditions and ensure patients' safety and comfort.

9. Documentation: Maintain accurate and up-to-date patient records, including progress notes, treatment plans, and outcomes.

10. Compliance: Adhere to all relevant laws, regulations, and organizational policies in the delivery of care.

Qualifications:

- Current Nurse Practitioner or Physician Assistant licensure in California

- Gerontological Nurse Practitioner or Physician Assistant with extensive Geriatric experience is a plus

- Minimum of 2 years of experience in geriatric or primary care

- Experience in a SNF setting is preferred

- Strong clinical assessment and diagnostic skills

- Excellent interpersonal and communication abilities

- Ability to work effectively in a collaborative, interdisciplinary team

- Proficiency in electronic health records (EHR) systems

- Compassion, patience, and a commitment to improving the lives of the elderly population

Compensation & Benefits

The compensation range listed is for starting base compensation only and is adjusted based upon years of experience and/or faculty rank: $150,000 - 175,000. This amount does not include variable compensation or extra productivity and is subject to the individual department compensation plans. More information on compensation is discussed with the departments during the recruitment process.
